I know it’s not that long that I have renew my passport, but sadly after visiting Hong Kong and Mainland China, the rats from the old house I was renting was having a mini buffet out of my passport that is still in my luggage. It is not that bad, but as I’ve asked DFA official social media accounts, they say I must renew it since it’s been Mutilated.
Since the first step in getting your passport renewed is getting a DFA passport renewal schedule online, let me show you how through this video.
Even though I showed how, I didn’t show you guys some samples to help you be able to understand more the whole thing. So here is some SAMPLE information to help you understand better.
PERSONAL INFORMATION
Assuming your name is CAMILLA DELA TORRE, who’s a legitimate (born within the marriage) child from Puerto Princesa Palawan. Here’s how you should enter your personal information.
FAMILY INFORMATION
This is more on about your parent’s information. Make sure it’s the same with the one in your birth certificate.
*Note: Mother’s maiden name means your mother’s name when she didn’t married your dad yet.
APPLICATION INFORMATION
This is now more about your application, are you wanting to get a new passport which means you’ve never been issued a passport ever before, or wish to renew your passport.
*Note:
NEW – if it’s a new passport, you won’t be asked for your passport details (e.g. passport number)
RENEW – as shown on the photo above you can find your passport number, issuing office, etc. on your passport that you wish to renew.
CONTACT INFORMATION
It’s basically your address, phone, and job. If you’re like me who’s working online and working freelance, you may leave the office address empty.
Once you confirm to go and pay, you’ll receive an email containing a REFERENCE NUMBER. Take note of that reference number and go to the nearest or preferred payment center for you. Once the payment is completed, you’ll then right away get an email with a file attached that you needed to print in an A4 size paper.
The payment is for your passport only, the scheduling is FREE. It’s just to ensure that all who wants a schedule are really those who want to get a new or renew their Philippine passport. Since before this implementation of having people pay first before getting a confirmed appointment at DFA for your passport, there has been a struggle to get an appointment.
For those who are like me who will be renewing a mutilated passport, there will be a penalty that will be paid at DFA.
